Previous keepers names and addresses"¦
Cheque or postal order for £5 made payable to DVLA Swansea SA991AJ
AND
A copy of a utility bill or bank statement issued to you within the last 3 months you should receive your reply within 4 weeks
possibly you all know this ....BUT just in case[;)]
