Telepeage - contrat de service

At the end of April I am signed up for the Driving Adventures trip to the South of France and thought it would be helpful to have the telepeage transponder that RobertC mentionned last year.
Accordingly I trawled back through the postings and found the website for downloading the forms but I am unclear whether the second sheet needs to be filled in at all or can be left blank?
Advice pls!

Sorry but a search for copies of my documentation has proved fruitless. I completed the on-line application forms so this may explain why I have no hard copies, although I do usually retain a print-out. The on-line process works fine so long as your French is up to it - I enlisted outside assistance. Well worth the effort though not just for the ease of avoiding the often long queues but also to spare my wife the humiliation of stretching out to the toll booth - I tend to leave a far too generous gap to avoid damaging the rims on the surrounding high kerb stones.
 
I used this on several trips last year. Well worth it for avoiding the worry of kerbing a rim. No hassle for the passenger any more. Avoid the queues (why don't the French themselves seem to use this?!)
