Tiptronic holds 3rd gear even though dash showing change to 4th

my '94 model Tiptronic S changes up and down through the box except when accelerating hard when it occasionally holds 3rd even though the dash indicator shows it's changed to 4th. Manually using the buttons on the wheel won't prompt it to change either. It has had a recent change of fluid and filter which hasn't made any difference. Any suggestions as to cause and cure?
