The more you describe your symptoms the more is sounds like hydraulic issues. Once the engine (and gearbox) get warm the heat will affect the hydraulics - exactly as hot brakes affects that. Air and water in the hydraulic system will expand with heat and reduce the effectiveness of the system.
I think I'd be inclined to do a full brake fluid change (it's supposed to be done every 2 years anyway) and obviously a full bleed. It's possible the seals in the slave cylinder are past their best but you'll only really know that once you've done the full bleed. IMHO, difficulty getting INTO gear is rarely a symptom of a worn clutch. Clutch slip is usually the giveaway there.
