Sorry haven't read the full post as I am on my way out, but could be either fuel pressure, injectors not pulsing, incorrect timing / ignition relay, fuel relay / pump or TPS. The key is process of elimination - rule out fueling, rule out ignition you are left with electrics and visa versa.
It will run and start with a faulty MAF, lambdas, crankshaft sensor, coil packs broken down (as not all will break down at the same time, will just run lumpy and fire on a few but still run), if your speed sensor is way out it won't fire up correcly. TPS won't pick up the throttle is open at all but should throw a fault, injectors probably won't throw a fault - I think the duty is measured through the ECU sending the signal not what they are actually doing.
